１．大学が皆さんといつでも連絡が取れるよう、以下のフォームに連絡先等の情報を入力してください。

２．大学の在留情報把握のため、次の手順に従い、５種類の身分証画像を提出してください。

①学生証（表）
②学生証（裏）
③在留カード（表）
④在留カード（裏） (最新の住所の登録があるもの)
⑤パスポート（顔写真とパスポート番号のあるページ）
★情報が全て写っていることを確認してから提出して下さい。
